When you change your signature, a screen loads up with a 'Signature Preview' and says something to the effect of "You've edited your forum signature! After checking it over, if you'd like to make changes, click the 'Edit' button or click 'Accept' to confirm the new signature."
This would, of course, be accompanied by the two aforementioned buttons. This would make for no need to make sigtest posts, etc.

Certain accounts are flagged to not use the forum's cache because they have advanced features. For example, blacklisted users never touch the cache, nor do accounts with the ability to see blacklisted posts.
Most people won't see an update to their sig until the forum's cache flushes for the thread, which requires a post or edit on the thread.
At least thats how I'm explaining my lack of ability to correctly click on the one I wanted, and not noticing I had misselected till I hit the vote button
It's not something people hear about.
