Recieved this spam e-mail today in my inbox, and as a deviation from my usual spam, this one had information on me such as my name and address. Thing is I rarely give this information out. Should I report this spam to my ISP?

You could just to be cautious. I wouldn't overly worry. Most of what they had seems to be a matter of public record. Unless they actually had your card number.

You'd be surprised how easy it is for these bastards to get info, even unlisted telephone numbers.
