Thats probably the most likely solution.
She also may have had a low crit strike, or its quite possible he/she wasnt QUITE in the righjt position to pull it off. thats happened to me

yep, that explains it. She likely hit it right as you were on top of her, but the game lagged a bit and you were through her when it actually went off
