I believe it's for opening the bottom level to get to the Ice Queen. It's been awhile since I've played the game.

If I remember right, after you got the music box and have found the entrance to the last level with the boss in it, you head back up to the city and go to that shop by the docks. The guy that was the only one left there at the beginning of the stage will be gone from his shop and a key will be lying about the countertop or floor, I believe

a little bit to add to post above. as far as i know. you have to go to fortune teller shop and click one of the options. to be able to see key in shop by docks

btw. have fun doing demon shaft *shivers* 20 hours...
