I have tiny ears. Doctors checking my ears with that oscilloscope (sp?) thing have to use the same insert they'd use for checking children's ears.
And for some reason, I have not spotted portable headphones marketed for children -- though if they existed, they'd sadly be covered in motifs for disney/toon characters 
Has anyone here found useful headphones for smaller ears? Please tell.

Might have some trouble finding etymotics small enough. CX300-B or MX55V perhaps? They both appear to be adjustable.

You asked for headphones, not earbuds. They're not the same thing.
Snarkiness aside, if your ears are as fucked up as you make them out to be, you're going to want to trundle down to your local audiophile shop and start trying things on until you find some that aren't excruciating. Here's a pretty good list of the things, but again, you're going to need to try them on.
I've heard that you can get custom tips made for some pairs, but that's apparently ridiculously expensive. Depends on how far you wanna go.

They're. To be fair, there are earbuds that sound decent (to my ears, at least), and it's not all that practical to wear your Grados on the go.

Vernal, I'd highly suggest picking up a pair of Shure E2C buds. They come with three different sizes of foam inserts, including a small one. They're like the foam used in earplugs -- compress it then stick it in your ear. Not only is the sound incredibly awesome, they should fit your ears. They do cost 99 bucks, but are worth every friggin cent you could spend on them.
